The process of moving a loved one into a nursing facility doesn’t have to be stressful for residents or their families. Hurlbut Care Communities strive to make the admissions process easy and uncomplicated.

Before admission to any Hurlbut Care Community, all residents will need to provide a PRI (Patient Review Instrument), NYS screening, medical history and physical, advanced directives and financial information. Other items may be needed depending on a resident’s diagnosis.

If transferring from a hospital, simply let the social worker know you are interested in one of our Hurlbut Care Communities. The social worker will obtain all of the information and fax it to the Hurlbut Care Community of your choice.

If coming to us from home, a PRI and screen will need to be completed, and information obtained from their physician in the community.

The Patient Review Instrument (PRI) is a medical evaluation tool that determines whether or not an individual is eligible for skilled nursing home placement, and it is mandated by law that a PRI be performed by a registered nurse. A PRI and Screen is valid in New York state for 90 days as long as the person’s health status does not change.

 We are utilizing Senior’s Choice Care Management to provide the preparation of Patient Review Instruments and Screens for individuals needing skilled nursing level of care coming from home, or other facilities. We are providing this resource FREE of CHARGE for those who need these required services for placement in one of our 13 skilled nursing facilities.

We encourage families to call the Hurlbut Care Community of their choice. You can speak with our admissions coordinator, Director of Social Work, or Administrator about your specific needs. We will be happy to assist you, answer any questions you may have, and determine if we can meet the needs of your loved one. We will notify either the hospital or you at home of our decision depending on where care is currently being provided. If we are able to provide care, we will work as quickly as possible to make the transition.
