Before admission to any Hurlbut™ Care Community, all residents will need to provide a PRI (Patient Review Instrument), screening, medical history and physical, advanced directives and financial information. Other items may be needed depending on a resident’s diagnosis.
If transferring from a hospital, simply let the social worker know you are interested in one of our Hurlbut™ Care Communities. The social worker will obtain all of the information and fax it to the Hurlbut™ Care Community of choice.
If coming here from home, a home care agency can complete the PRI and screen, and obtain the other information from the physicians in the community.

We encourage families to take a personal tour of the Hurlbut™ Care Communities of their choice. You can call in advance to schedule an appointment with an admissions coordinator or stop in at your convenience. Our administrative staff members will be happy to assist you. They can also review the referral information and determine if we can meet the needs of your loved one. We will notify either the hospital or you at home of our decision depending on where care is currently being provided. If we are able to provide care, we will work as quickly as possible to transition the your loved one.
