It happens. An injury or acute illness finds you or a family member in the hospital. And while everything turns out fine, sometimes, before you or your family member can return home, there is a need for support, retraining and rehabilitation to help regain strength, mobility, communication skills and overall independence for life’s daily activities.
Hurlbut Care Communities offer the highest quality short-term rehabilitative care available to help patients get better, faster.
Every one of our 12 Hurlbut Care Communities – located throughout the Greater Rochester region – offers a specialized unit dedicated to providing comprehensive rehabilitation services. Two of our communities, The Brightonian on Elmwood Avenue and Woodside Manor on Clinton Avenue South, offer rehabilitative care seven days per week.
Hurlbut Care Communities admit patients 24/7/365, so there is no lapse in care, and so rehabilitation services can start immediately.
Our Primary Rehabilitation Services include:
- Occupational Therapy: focuses on the improvement of a patient’s self-care independence by restoring function skills and developing effective compensatory strategies for people with mental, physical, emotional, or developmental impairments.
- Physical Therapy: works on improving a patient’s ability to move, reduce pain, and help restore posture, balance, range of motion and strength, through the use of therapeutic exercise and physical modalities.
- Speech Therapy: focuses on improving a patient’s speech, language, communication and oral motor skills.
- Orthopedic Rehabilitation: provides therapy as part of the after-care treatment related to disease, injury and surgical procedures to bones, muscles, joints and tendons – such as hip and knee replacements.
- Neurological Rehabilitation: especially for people who have limitations from stroke, Parkinson’s disease, Multiple Sclerosis and paralysis; provides treatment of conditions affecting the central nervous system (brain and spinal cord).

Personal Plan of Care
A customized plan of care.
Within hours of checking in to a Hurlbut Care Community, patients undergo a thorough Physical and Occupational Therapy Evaluation to determine their specific rehabilitation needs. If needed, a Speech Therapy Evaluation is also conducted.
Then, our rehabilitation team works closely with the physician team and the patient’s family to develop a personalized plan of care that establishes goals and formalizes a treatment program specific to the patient’s needs.
Unlike other rehabilitation facilities, at Hurlbut Care Communities treatment begins on the day of admission and can continue seven days a week, or in accordance with a customized treatment plan. Family and friends are welcome to observe and offer encouragement to patients during their rehabilitation services.
Communication is key. The rehabilitation team will meet with the patient and their family within two weeks of admission to discuss progress toward stated goals and the ongoing treatment plan. The Hurlbut staff will also help schedule follow-up appointments with primary care professionals and specialists, as needed, while ensuring that the patient’s entire healthcare team is up to date on the progress of the rehab plan.

Skilled Professionals
A team of skilled therapy professionals.
Our therapists are trained and experienced in managing the rehabilitation plan of those recovering from an acute illness, an exacerbation of a chronic illness or a surgical procedure. In addition, training in a diverse spectrum of clinical disciplines including oncology, orthopedic, cardiopulmonary and neurological disorders, enable our rehabilitation team to manage nearly any therapeutic need.
Our skilled therapy practitioners utilize traditional and state-of-the-art treatment techniques and technologies to deliver the most effective, most efficient therapy to our patients. Adaptive equipment is also provided to help patients function at the highest level.
It’s important to realize that our rehabilitation therapists are not contract or temporary employees, as at some other rehabilitation facilities. Each of them is employed full-time by Hurlbut Care Communities. This fosters a level of personal dedication and commitment unmatched by most other short-term rehabilitation facilities.
